Immediate safety
If a patient is in immediate danger, we will call 999 without delay. Patient safety takes priority over all other considerations.
Listen and record
We will listen carefully to the patient, avoid leading questions, and document concerns accurately and promptly in the clinical record using the patient's own words where possible.
Do not investigate
It is not our role to investigate safeguarding concerns. We will refer to the appropriate statutory authority and not attempt to carry out our own investigation.
Refer to statutory services
We will make a referral to the relevant local authority Adult Social Care team or, where a child is at risk, to Children's Services. We will follow the local safeguarding board procedures.
Inform the patient
Where it is safe to do so and will not increase risk, we will inform the patient that we are making a referral and explain why. We will act in the patient's best interests at all times.
Document and review
All safeguarding actions are documented in the clinical record. We will follow up to ensure the referral has been received and acted upon.
Safeguarding concerns may require us to share information without the patient's consent. The principle of confidentiality is not absolute — where there is a serious risk of harm to a patient or another person, the duty to protect life overrides the duty of confidentiality.
We follow the Caldicott Principles and the Information Governance Alliance guidance when making decisions about sharing information for safeguarding purposes. We will share only the minimum necessary information with the appropriate authorities.
We follow the Mental Capacity Act 2005 in all clinical interactions. We assume that every patient has capacity to make decisions about their own care unless there is evidence to the contrary. Where capacity is in doubt, we carry out a formal capacity assessment and, if the patient lacks capacity, act in their best interests following the Act's principles.
We are alert to situations where a patient may be subject to undue influence or coercion that affects their ability to make free and informed decisions about their care.
Our Nurse Consultant maintains up-to-date safeguarding training in line with the NMC requirements and the Intercollegiate Safeguarding Competency Framework. Safeguarding training is reviewed and updated regularly.

This safeguarding policy is reviewed annually or following any significant safeguarding incident, change in legislation, or CQC guidance. The next scheduled review date is March 2027.
