Safeguarding Policy

1. Purpose and Scope

LSJ Rejuvenate Ltd is committed to safeguarding the welfare of all patients, particularly those who may be vulnerable due to age, disability, mental health, or other circumstances. This policy applies to all clinical and administrative activities carried out by LSJ Rejuvenate Ltd.

This policy is written in accordance with:

  • CQC Fundamental Standard: Regulation 13 — Safeguarding Service Users from Abuse and Improper Treatment
  • Care Act 2014 — adult safeguarding duties
  • Mental Capacity Act 2005 — protecting those who lack capacity
  • NMC Code of Conduct — professional obligations of our Nurse Consultant
  • Human Rights Act 1998

2. Our Safeguarding Commitment

LSJ Rejuvenate Ltd is committed to:

  • Providing care that promotes the dignity, safety, and wellbeing of every patient
  • Recognising and responding appropriately to signs of abuse, neglect, or exploitation
  • Ensuring that no patient is subjected to abuse, discrimination, or degrading treatment
  • Acting promptly when a safeguarding concern is identified
  • Working collaboratively with statutory agencies where necessary
  • Maintaining appropriate confidentiality while prioritising patient safety

3. Types of Abuse We Are Alert To

Our Nurse Consultant is trained to recognise the following categories of abuse and neglect:

Physical abuse
Emotional / psychological abuse
Sexual abuse or exploitation
Financial or material abuse
Neglect and acts of omission
Discriminatory abuse
Institutional or organisational abuse
Modern slavery and trafficking
Domestic violence and coercive control
Self-neglect

4. Identifying a Safeguarding Concern

Indicators that a safeguarding concern may exist include, but are not limited to:

  • Unexplained or inconsistent injuries
  • Signs of significant weight loss, malnutrition, or self-neglect
  • Expressions of fear, anxiety, or distress
  • Disclosure of abuse by the patient
  • Controlling behaviour by a third party accompanying the patient
  • Evidence of financial exploitation
  • Concerns about mental capacity or undue influence in decision-making

5. How We Respond to a Safeguarding Concern

Immediate safety

If a patient is in immediate danger, we will call 999 without delay. Patient safety takes priority over all other considerations.

Listen and record

We will listen carefully to the patient, avoid leading questions, and document concerns accurately and promptly in the clinical record using the patient's own words where possible.

Do not investigate

It is not our role to investigate safeguarding concerns. We will refer to the appropriate statutory authority and not attempt to carry out our own investigation.

Refer to statutory services

We will make a referral to the relevant local authority Adult Social Care team or, where a child is at risk, to Children's Services. We will follow the local safeguarding board procedures.

Inform the patient

Where it is safe to do so and will not increase risk, we will inform the patient that we are making a referral and explain why. We will act in the patient's best interests at all times.

Document and review

All safeguarding actions are documented in the clinical record. We will follow up to ensure the referral has been received and acted upon.

6. Confidentiality and Information Sharing

Safeguarding concerns may require us to share information without the patient's consent. The principle of confidentiality is not absolute — where there is a serious risk of harm to a patient or another person, the duty to protect life overrides the duty of confidentiality.

We follow the Caldicott Principles and the Information Governance Alliance guidance when making decisions about sharing information for safeguarding purposes. We will share only the minimum necessary information with the appropriate authorities.

7. Mental Capacity

We follow the Mental Capacity Act 2005 in all clinical interactions. We assume that every patient has capacity to make decisions about their own care unless there is evidence to the contrary. Where capacity is in doubt, we carry out a formal capacity assessment and, if the patient lacks capacity, act in their best interests following the Act's principles.

We are alert to situations where a patient may be subject to undue influence or coercion that affects their ability to make free and informed decisions about their care.

8. Training and Competence

Our Nurse Consultant maintains up-to-date safeguarding training in line with the NMC requirements and the Intercollegiate Safeguarding Competency Framework. Safeguarding training is reviewed and updated regularly.

10. Policy Review

This safeguarding policy is reviewed annually or following any significant safeguarding incident, change in legislation, or CQC guidance. The next scheduled review date is March 2027.
