About this document: This Statement of Purpose is produced in accordance with the Health and Social Care Act 2008 and the Care Quality Commission (CQC) registration requirements. It sets out the aims, objectives, regulated activities, and service model of LSJ Rejuvenate Ltd as a CQC-registered provider of Treatment of Disease, Disorder or Injury (TDDI).

2. Aims and Objectives

LSJ Rejuvenate Ltd aims to provide high quality, safe, and professional weight management services to our patients, delivered by trained, qualified, and experienced staff.

We aim to achieve high standards by:

  • Providing safe, professional, and effective remote and in-person weight management services, tailored to each patient's needs.
  • Offering initial consultations remotely (video or telephone) to ensure accessible, convenient entry into our services, followed by face-to-face suitability assessments in the patient's own home for personalised care and safe clinical decision-making.
  • Focusing on the promotion of health and wellbeing, preventing illness where possible, and offering practical care and advice to support safe and sustainable weight loss.
  • Working in partnership with our patients, their families, carers, and other healthcare professionals to ensure a positive experience and involve them fully in decisions about their care and treatment.
  • Ensuring effective communication of patient information where relevant and appropriate, enabling joined-up care and reducing duplication.
  • Being a learning organisation that listens to patients, partners, and staff to continuously improve the services we offer.
  • Treating patients as individuals, with dignity and respect, supporting them to express their needs and preferences, and enabling them to maintain independence, choice, and control.
  • Working collaboratively with other agencies and healthcare professionals where appropriate, ensuring valid consent is obtained and patients only need to share their story once.
  • Encouraging patients and staff to provide feedback so that we can make improvements and enhance outcomes.
  • Ensuring all staff are trained, competent, and motivated to deliver care to a high standard, maintaining up-to-date skills and knowledge.
  • Taking proactive measures to ensure equitable access to our services, including support for patients whose first language is not English or those with sight or hearing impairments.
  • Offering alternative options, such as home visits and remote consultations, to ensure services are accessible to all patients.
  • Protecting both staff and patients against abuse, operating a zero-tolerance approach to all forms of abuse.
  • Maintaining a programme of regular audit, monitoring, and quality assurance to ensure services remain safe, effective, and compliant with legislation, clinical guidelines, and best practice.
  • Operating on a financially sound basis to ensure continuity of safe and effective services.

4. Regulated Activity

Regulated Activity: Treatment of Disease, Disorder or Injury (TDDI) — as registered with the Care Quality Commission under the Health and Social Care Act 2008.

As a private clinic registered with the CQC, LSJ Rejuvenate Ltd provides services under the regulated activity of Treatment of Disease, Disorder or Injury (TDDI). Our core focus is supporting patients with weight management, obesity, and related conditions such as prediabetes.

1. Medical Weight Management

  • Comprehensive Assessments: Patients initially undergo a structured remote consultation (video or telephone), which includes identity verification, medical history, risk assessment, and informed consent.
  • Face-to-Face Suitability Assessments: Following the remote consultation, a clinician will visit the patient's home to complete a physical assessment, confirm suitability, and ensure safe treatment planning.
  • Individualised Plans: Each patient receives a tailored treatment plan that may address associated conditions such as prediabetes, hypertension, and PCOS.
  • Evidence-Based Care: All management is guided by clinical best practice and evidence-based approaches, incorporating lifestyle support, coaching, and behaviour change techniques.

2. Prescribing and Monitoring

  • Medicines: Licensed weight loss medications such as semaglutide (Wegovy/Ozempic), liraglutide (Saxenda), tirzepatide (Mounjaro), and orlistat may be prescribed if clinically suitable.
  • Clinical Oversight: Prescribing will only take place following both the remote consultation and the in-home suitability assessment.
  • Monitoring: Patients are monitored regularly through remote reviews (telemedicine) and, where necessary, in-person follow-ups to ensure treatment safety, efficacy, and patient wellbeing.

3. Remote and In-Person Consultations

  • Secure Telemedicine: All online consultations are conducted via secure platforms, following GMC and CQC guidance, and include triage, identity verification, history-taking, and thorough documentation.
  • Accessibility: Remote consultations provide convenience and accessibility, while in-home suitability assessments ensure safe, patient-centred clinical care.
  • Continuity of Care: Ongoing support is delivered through a blended model of remote and in-person consultations, ensuring patients are fully supported throughout their treatment journey.

5. Registered Manager

The Registered Manager of LSJ Rejuvenate Ltd holds overall responsibility for the delivery of regulated activities and compliance with CQC requirements.

6. Service Location & Delivery Model

LSJ Rejuvenate Ltd delivers its regulated activity of Treatment of Disease, Disorder or Injury (TDDI) through a home-based clinical office used solely for remote consultation purposes. The home office is set up in line with information governance, confidentiality, and digital safety standards to ensure secure delivery of telemedicine services.

  • Remote Consultations: All initial and follow-up consultations are provided from the registered home office using secure, compliant telemedicine platforms. Patients do not attend this location in person.
  • Face-to-Face Suitability Assessments: Where a physical examination or suitability check is required, these are carried out in the patient's own home. This enables safe, personalised assessment and supports accessibility for patients who may find it difficult to attend a clinic.
  • Service Model: The combination of a secure home-based office for digital consultations and in-home patient assessments ensures that care is delivered safely, effectively, and in line with CQC requirements.

7. Service User Groups
